Comprehensive Care Management Services
The goal of care management services is to ensure individuals who have complex physical and/or behavioral health needs get access to the care and services they need in order to be healthy. 800-853-6104
Care Management Services Include But Are Not Limited To:
- Help understanding chronic conditions and symptoms
- Assistance accessing needed care and services
- Coordination of care
- Advocacy at appointments
- Transportation to and from appointments
- Assistance of Medicaid/county paperwork
- 24 hour crisis support
Eligibility:
- Medicaid or Medicaid Managed Services recipient or Medicaid eligible
- 2 or more Chronic Medical or Mental Health Conditions*
- Or have 1 of the following Chronic Conditions:
- HIV/AIDS
- Complex Trauma
- SED (Serious Emotional Disturbance)
